As a Registered Nurse, you will play a vital role in our interdisciplinary team, making a positive impact on the lives of our patients every day. Primary Responsibilities:
- Plan of Care:
- Conduct case assessments and participate in developing patient care plans. Manage interdisciplinary care and promote continuity of care.
- Supervision:
- Oversee patient care provided by Home Health Aides and Licensed Practical Nurses.
- Education:
- Educate patients and families about disease processes and care plans.
- Communication:
- Effectively communicate with the interdisciplinary team, patients, and families regarding all aspects of care.
- Documentation:
- Complete all clinical documentation accurately and promptly to ensure optimal care delivery.
