Primary Responsibilities:

• Plan of Care:
• Conduct case assessments and participate in developing patient care plans. Manage interdisciplinary care and promote continuity of care.
• Supervision:
• Oversee patient care provided by Home Health Aides and Licensed Practical Nurses.
• Education:
• Educate patients and families about disease processes and care plans.
• Communication:
• Effectively communicate with the interdisciplinary team, patients, and families regarding all aspects of care.
