Job Responsibilities:

• Help individuals maintain, restore, and improve movement, activity, and function to enable optimal performance and enhance health, well-being, and quality of life, under a physician-approved plan of care, supervised by a Physical Therapist.
• Provide physical therapy services and interventions under the supervision of a Physical Therapist, in accordance with the plan of care.
• Document each visit according to guidelines and regulatory standards for inclusion in the patient’s clinical record.
• Observe, record, and report the patient’s response to treatment and any changes in condition to the supervising PT, physician, and clinical staff.
• Provide resources for future needs and promote wellness and prevention.
• Work under supervision of the Physical Therapist, following applicable regulations.

Requirements:

• License in the State of Virginia
• Prior home care experience or strong clinical background
• Prior EMR experience (Kinsser preferred)
• Must be efficient with documentation
