Responsibilities
  • Responsible for providing professional nursing care to patients in their home through assessment, development, implementation and evaluation of home nursing care needs on an individual and continuous basis in accordance with organization policies, procedures and external regulations.
  • Assure compliance with appropriate physician orders in a timely manner and that visit frequency follows physician orders. Ensure documentation reflects compliance with physician's orders and progression toward goals at discharge.
  • Ensure that assessment, planning, intervention and evaluation of patient care follow policy, procedure, and standard of care. Demonstrate holistic care for patient and significant other. Ensure safe patient care and competency in assigned treatments and procedures; utilize interdisciplinary team and accurately document all activities.
  • Ensure clinical judgments and critical thinking reflect professional standards of care and codes of ethics to provide safe and effective patient outcomes. Document clinical decisions appropriately.
  • Identify appropriate payors and remain in compliance with authorizations for self and other disciplines as necessary. Meet payor source guidelines. Maintain visit frequency within authorization range. Utilize secure messaging to insurance nurse for all authorizations.
  • Document assessment, interventions and planning to meet organization timelines. Complete nursing assessment, medication reconciliation and collaborate with physician on Plan of Care. Update care plan within two weeks of a change in patient status.
  • Implement teaching strategies to meet goals on the plan of care. Document teaching strategies in the medical chart aligned with goals and plans.
  • Report patient care problems to manager or appropriate nursing management personnel and/or physician in a timely manner with possible solutions. Document case communication and use the manager as a resource.
  • Act as liaison among physician, Manager of Clinical Practice and other health care team members to ensure effective coordination and appropriate resource referrals. Ensure coordination of services meets client needs.
  • Communicate pertinent patient information and Plan of Care during transfers of patient care loads, and when vacations or time off are scheduled.
  • Complete routine assignments accurately and independently in a timely manner without supervision. Meet productivity requirements consistently.
  • Act as clinical supervisor of para-professional home health aide staff. Complete supervisory visits within regulatory time frames and document visits. Participate in the evaluation of home health aide staff.
  • Maintain awareness of new and updated standards and incorporate these into nursing practice across the continuum of care. Document knowledge of current practices and guidelines in the medical chart.
  • Other duties and/or projects as assigned. Demonstrate flexibility with job responsibilities in all areas.
  • Adhere to organizational competencies and standards of behavior.
  • Lifts a minimum of 50 lbs., pushes and pulls a minimum of 50 lbs., and stands for a minimum of 8 hours a day.
Qualifications Education, Knowledge,

Skills and Abilities

Required
  • Associate's Degree in Nursing or Diploma Certificate in Nursing
  • At least six (6) months RN experience or at least 1 year of LPN experience, preferably in home care or hospice
  • PC Experience/Skills

  • NJ State Professional Registered Nurse License
  • AHA Basic Life Support for Healthcare Providers (BLS HCP)
Compensation

Starting at $88,441.60 Annually

HMH is committed to pay equity and transparency for our team members. The posted rate of pay is a reasonable good faith estimate of the minimum base pay for this role at the time of posting in accordance with the New Jersey Pay Transparency Act.
