Position Overview:
We are seeking Board-Certified Family Nurse Practitioners to join our dedicated healthcare team on a locum tenens basis. This is a unique opportunity to provide high-quality patient care in a home healthcare setting, serving communities across Ocean County, NJ.
Key Responsibilities:
Conduct 5???6 home health visits per day Perform annual risk assessments and review chronic conditions Escalate urgent findings to the patient???s primary care provider Perform A1C tests when applicable (no prescribing or treatment required) Utilize company-provided equipment (iPad, BP cuff, lab kits)
Qualifications:
Board Certification in Family Practice (NP) Active New Jersey medical license and NJ Medicare number Previous home healthcare experience preferred Shift Information: 8-hour shifts with 1-hour lunch (e.g., 8 AM ??? 5 PM, 12 PM ??? 8 PM) Flexible shift options within Monday???
