Physical Therapist is responsible for physical therapy examination, evaluation, intervention planning and implementation, patient/client family instruction, and outcomes analysis within the scope of practice as defined by the State Department of New Jersey Division of Consumer Affairs. The PT is engaged in coordination, communication and documentation to ensure comprehensive, individualized and evidence-based services. Throughout care, the PT ensures optimal functional independence, prevention of recurrence, and promotion of wellness-oriented lifestyle.
Settings may include inpatient (acute and rehab) and/or outpatient.
Position is day shift, Monday - Friday, no holidays or weekends.
Responsibilities- Prepares for the evaluation in accordance with department policies/procedures and standards of practice including obtaining patient history, performing systems review exhibiting knowledge of patient diagnoses and researching unfamiliar diagnoses.
- Selects and administers tests and measures and performs system review. Additionally, synthesizes evaluation results to formulate the diagnosis, goals, prognosis and plan of care.
- Establishes a plan of care based on examination and evaluation results which includes person centered goals and best practice. Collaborates with the interdisciplinary team, patient/clients and patient's/client's support system.
- Assess and re-assess need for modifications to plan of care and goals.
- Identifies need for consultations and/or refers the patient/client to another health care professional or health care services.
- Initiates and completes discharge plan.
- Provides patient/client instruction to inform, educate and/or train patients/clients and their support systems.
- Provides and selects interventions based on the PT's assessment of the client's current condition which can include but is not limited to: patient and client instruction; airway clearance techniques; prescription, application, and, as appropriate, fabrication or modification of assistive technologies; implementation of biophysical agents; therapeutic exercise; motor/functional training; integumentary repair and protection; manual therapy techniques; functional training in self-care, domestic life, community and work integration and reintegration.
- Collects and records patient care data that follows format and guidelines of the patient care service, regulatory agencies and third‑party payers which reflects an understanding of reimbursement systems and their impact on patient care. This includes, but is not limited to, initial evaluation, daily notes, reassessments/progress reports, discharge plans and communication with interdisciplinary members of the team.
- Records daily billing and follows department guidelines for productivity, as well as adheres to insurance and state regulations on the billing of services rendered.
- Provides appropriate direction and supervision to other personnel as assigned.
- Lifts a minimum of 50 lbs., pushes and pulls a minimum of 50 lbs. and stands a minimum of 1 hour a day.
- Other duties and/or projects as assigned.
- Adheres to HMH Organizational competencies and standards of behavior.
- Graduate of a CAPTE accredited Physical Therapy Program or equivalent.
- Excellent written and verbal communication skills.
- Proficient computer skills that may include but are not limited to Microsoft Office and/or Google Suite platforms.
- Minimum of 1 year of experience.
