Overview
Manager Clinical Practice Homecare-Iselin, NJ-FT at Hackensack Meridian Health. This role oversees the development and implementation of the physician’s plan of treatment and supervises clinical staff and appropriate support staff. Ensure ongoing performance improvement activities and regulatory compliance.
Responsibilities- Oversee the development and implementation of the physician’s plan of treatment. Assure services are in compliance with physician orders. Ensure the plan of treatment is received within regulatory time frames and re-certifications are timely, complete, and accurate.
- Conduct regular case conferences/team meetings with staff. Perform supervisory visits within organization and regulatory requirements. Match client with staff according to needs, territories, client loads and clinical skills.
- Participate in recruitment and orientation of staff. Assure appropriate staffing. Maintain vacancy and turnover metrics at established goals.
- Review client records to ensure care is appropriate. Ensure documentation supports care delivered. Manage daily operations through the reporting system.
- Support staff’s understanding of nursing standards, regulatory policies and organization policies. Ensure staff are compliant with all mandatory in-services.
- Perform ongoing performance improvement activities. Measure PI indicators quarterly with action plans. Complete clinical record reviews and trending reports. Maintain infection logs and incident reports.
- Maintain relationships with referral sources, community, patients, families, and subcontractors. Handle complaints per policy. Review complaint logs quarterly and develop appropriate resolutions. Respond to referral sources promptly. Aim for customer satisfaction at or above benchmarks.
- Identify payors and ensure accurate and timely billing within regulatory time frames. Obtain authorizations for visits. Ensure PPS matrix and visit frequency align with 485 form. Bill supplies appropriately.
- Recognize patient/client grievances and respond constructively. Ensure customer satisfaction is at or above benchmarks.
- Recognize and address team member issues professionally. Monitor turnover rates against benchmarks.
- Prepare materials for team meetings and conduct meetings in the absence of the Nurse Manager.
- Remain available to clinicians after hours for concerns/questions.
- Adhere to organizational competencies and standards of behavior.
- Maintain physical requirements as applicable (e.g., lifting, standing as described in job documentation).
- Three (3) to Five (5) years RN experience
- Proficient in Microsoft Office Suite
Skills and Abilities
Preferred
- BSN in progress and/or willing to acquire within three (3) years of hire
- Two (2) to three (3) years of management experience
- Homecare experience
- New Jersey State Professional Registered Nurse License
- American Heart Association Basic Life Support Healthcare Provider Certification
