-Ageing Well Team - Location: Wolverhampton
- Salary: £31.97 ph
We are recruiting an Experienced Social Worker to join the Ageing Well Team in Wolverhampton. This is an excellent opportunity to work within a dedicated adult social care service supporting older adults to maintain independence, safety, and wellbeing within the community.
Key Responsibilities- Manage a caseload of older adults with complex needs.
- Undertake Care Act assessments, reviews, and support planning.
- Complete mental capacity and best interest assessments where required.
- Carry out safeguarding enquiries and risk assessments.
- Promote strengths-based practice and independence.
- Work collaboratively with health professionals, community services, and families.
- Support timely hospital discharges and prevent unnecessary admissions where applicable.
