Job summary

We have an exciting opportunity for an Advanced Clinical Practitioner to join our Frailty Team at the West Midlands Regional Hub Office based at HMP Oakwood Visitors Centre. As an Advanced Clinical Practitioner you will provide high‑quality, person‑centred care across three sites: HMP Oakwood, HMP Featherstone and HMP Brinsford. The ideal candidate will have experience and a genuine interest in supporting individuals with long‑term conditions, demonstrating empathy, patience, and a person‑centred approach.

They will be able to understand and respond to a range of physical, mental or chronic health needs, promoting independence, dignity, and wellbeing at all times.

Hours
  • Monday – Friday
  • 37.5 hours per week
  • 8am – 4pm
Salary

This is a full‑time position. You will receive an annual salary between £59,677 and £66,791 per annum FTE, depending on experience.

Main duties of the job

As an Advanced Clinical Practitioner, your responsibilities will vary. You will

  • Commit to the development and delivery of a frailty framework
  • Complete Comprehensive Geriatric Assessments (CGA) demonstrating critical thinking in a safe clinical decision‑making process
  • Provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care
  • Demonstrate safe, competent expert holistic care, including assessment and diagnostic skills
  • Communicate and work collaboratively with the interdisciplinary teams across sites to meet the needs of patients, supporting the delivery of the Frailty framework and providing clinical education and leadership, upskilling the local workforce
  • Demonstrate all four pillars of advanced clinical practice: clinical practice, leadership, management, education and research

Qualifications required
  • Registered Nurse (NMC)
  • Independent/supplementary prescriber qualification
  • MSc in Advanced Practice
  • Experience working autonomously as an advanced clinical practitioner following best practice guidance
