📍 Location: Wokingham (Office-Based)
💰 Salary: £40,000 – £45,000 DOE
🚨 We’re recruiting for an experienced Care Coordination Manager to lead and oversee a high‑performing coordination function within a growing complex care provider. This is a pivotal operational leadership role, responsible for workload allocation, rota governance, package mobilisation, and stakeholder engagement across complex and lower‑risk care packages.

- 🔹 Lead and manage a team of Care Coordinators, allocating workload across complex and lower‑risk packages
- 🔹 Oversee rota planning, workload balancing, and SLA‑driven task management
- 🔹 Ensure proactive scheduling, forward planning, and visibility of care delivery
- 🔹 Act as the main scheduling contact for new package mobilisation and transitions
- 🔹 Work closely with recruitment, clinical, and operations teams to match carers to packages
- 🔹 Monitor compliance with care records, data entry, and documentation standards
- 🔹 Represent the coordination function at MDT meetings and liaise with commissioners and stakeholders
- 🔹 Track and report key operational metrics including rota gaps, mobilisation timelines, and team performance
- 🔹 Conduct performance reviews, coaching, and team development
- 🔹 Identify process improvements and lead service optimisation initiatives
- 🔹 Participate in the on‑call rota on a rotational basis
- • Minimum 4 years’ experience in care coordination, operations, or workforce planning
- • Proven people management and leadership experience
- • Strong knowledge of rostering, payroll, and care management systems
- • Excellent organisational, communication, and stakeholder engagement skills
- • Calm, solution‑focused approach under pressure
- • Full UK driving licence
- • NVQ/QCF Level 3+ in Health & Social Care or Management
- • Understanding of CQC regulations and complex care delivery
- • Salary £40,000 – £45,000 DOE
