Role Overview

Senior Physiotherapist (Band
6) with the clinical skills, expertise and knowledge to support community reablement, 2‑day urgent, community Physio & Falls service.

Intermediate Care is a multidisciplinary service that helps people to be as independent as possible. It provides support and rehabilitation to people at risk of hospital admission or who have been in hospital. It aims to ensure people transfer from hospital to the community in a timely way and to prevent unnecessary admissions to hospitals and residential care.

The post involves 7 days working within a shift pattern within the intermediate service.

Responsibilities
  • Complete the timely assessment and management of referrals with multiple co‑morbidities and complex rehabilitation needs.
  • Support triage of referrals for intermediate care services.
  • Provide equipment to promote independence and safety.
  • Hold a caseload and work autonomously.
  • Work flexibly to ensure urgent referrals are adequately managed.
  • Support colleagues in the delivery of compassionate, safe and structured care.
  • Provide guidance and appropriate delegation within the team.
  • Participate in service development and quality improvement.
  • Support the supervision of more junior members of staff and provide education and clinical physiotherapy advice to staff and students in Intermediate Care.
