Role Overview
Connect to Community (C2C) is a key part of how we deliver resident‑focused, strengths‑based adult social care. As a Senior Social Care Assistant in C2C you will be the first point of contact for residents, carers and professionals, working within a multi‑disciplinary team to provide timely, proportionate and joined support. You will hold strengths‑based conversations, triage safeguarding concerns, apply the Section 42 test, manage risks, and ensure cases remain within C2C until they are stable.
Key Responsibilities- Respond to new contacts and support residents during periods of change or crisis.
- Gather relevant information, provide advice and guidance, and signpost to appropriate community and universal services.
- Triage safeguarding concerns, apply the Section 42 three‑part test and take prompt action to manage immediate risks.
- Carry out welfare visits and Mental Capacity Act work where required.
- Collaborate with colleagues across reablement, technology‑enabled care, occupational therapy and voluntary services.
- Use judgement to balance risk, proportionality and positive outcomes for residents, working within agreed time frames to prevent unnecessary delays.
- Experience working in adult social care and a good working knowledge of the Care Act and Mental Capacity Act.
- Confidence in gathering information, managing risk and making balanced decisions.
- Ability to work effectively in a busy, changing environment while maintaining a person‑centred approach.
- Excellent communication skills, able to engage clearly and professionally with residents, carers and partner agencies.
- Valid UK driving licence and access to a vehicle to travel across Surrey (urban and rural areas).
- Enhanced DBS and Children’s/Adults’ Barred List checks will be required.
- 36‑hour working week, starting salary £33,552 per annum.

Surrey County Council is undergoing Local Government Reorganisation, moving from a two‑tier system to two new unitary councils in April 2027. If employed on 1st April 2027, the role will transfer with current terms and conditions to one of the new organisations.
