Position: Licensed Practical Nurse (LPN)

Licensed Practical Nurse (LPN)
Woburn Nursing and Rehab Center – Woburn, MA
Full-Time | Part-Time | Per Diem | Various Shifts

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our nursing team. In this role, you will provide direct resident care, supervise nursing support staff, and ensure high-quality, resident-centered care in accordance with facility policies and regulatory requirements.

Responsibilities
  • Direct the daily activities of Certified Nursing Assistants and other nursing personnel on assigned units
  • Ensure compliance with facility policies, procedures, and federal/state regulations
  • Participate in admissions, discharges, and transfers of residents
  • Observe, document, and report changes in residents’ status to physicians, families, and leadership
  • Transcribe and carry out the physician s orders accurately
  • Prepare and administer medications and monitor for adverse effects
  • Perform wound care, colostomy changes, tube feedings, suctioning, IV administration, and other assigned skilled nursing interventions
  • Conduct rounds to ensure resident needs are met, and staff are performing their duties correctly
  • Collaborate with the interdisciplinary care team to ensure holistic care for residents
  • Initiate, review, and update resident care plans as needed
  • Provide end-of-life care as required
  • Maintain adequate stock of medications, supplies, and equipment; notify appropriate personnel of needs
  • Promote teamwork, mutual respect, and effective communication
  • Participate in surveys, inspections, and quality assurance activities
Qualifications
  • Current, unencumbered Massachusetts LPN license
  • Strong communication and critical-thinking skills
  • Ability to handle situations with tact and professionalism
  • Prior long-term care experience preferred but not required
