Licensed Practical Nurse Part Time | Day, Evening and Night Shifts

** PAY RATE BETWEEN $31-36.75/HOUR **

(based on experience and shift differentials)

• Valid Licensed Practical Nurse (LPN) license in the state of Wisconsin.
• Current and valid CPR certification.
• Experience in long-term care is preferred.
• Basic computer proficiency.
• Ability to frequently push or pull objects weighing up to 60 pounds and occasionally lift and move objects weighing 100 pounds or more.

• Safely provide medications and treatments as prescribed by physicians, following accepted standards of practice and organizational policies.
• Maintain accurate, timely written and electronic records in compliance with legal, regulatory, and company requirements.
• Monitor and document residents' conditions, report changes to the RN and relevant team members, and collaborate in developing and implementing individualized care plans.
• Assist with admissions and readmissions, gather clinical data, support resident care programs by ensuring adherence to plans of care, and oversee CNA performance in these areas.
