Status: Full‑Time - This position is typically Monday through Friday, day shift hours, with some weekends and holidays.

Job Summary:The MDS Coordinator role at Edgerton Care Center requires someone with a minimum of one year of experience in care planning, assessment and is responsible for the overall completion of each resident's MDS, RAI and assessment. Critical thinking, leadership skills flexibility, excellent communication skills, and a focus on team building are a must viding quality care while establishing trust and building lasting relationships with our residents and their families are the values we possess at Edgerton.

Compensation:- $64,000.00 - $75,000.00 annual salary if LPN licensed - wage dependent on experience and education
- $85,000.00 - $98,000.00 annual salary if RN licensed - wage dependent on experience and education

Key Responsibilities:- Utilizes pertinent documentation obtained from the medical record to accurately complete MDS/RAI and assessments and transfer the correct information to the MDS document.
- Observant for significant changes in resident’s condition for the purpose of the MDS.
- Maintains and monitors individual MDS/RAI to maintain an optimal reimbursement level.
- Completes all necessary and required forms/documentation in a timely manner.
- Performs the role of a clinical manager as necessary (applies to RN’s only).
- Other duties:
Completes all other duties as assigned.

Qualifications:Required:
- Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in Wisconsin (required) or licensed in any U.S state and be eligible for Wisconsin licensure by reciprocity.
- Long‑term care or skilled nursing experience
- Knowledge of current laws and regulations regarding skilled nursing and long‑term care
- Must be English literate and Excellent communication, organizational, and problem‑solving skills
- Ability to meet physical and job‑related requirements
- AAPACN certification (eligible for increased wage)
