Position Overview

This field-based role enables associates to primarily operate in the field, traveling to client sites or designated locations as required, with occasional office attendance for meetings or training. The role provides flexible, responsive, hands‑on engagement to meet member needs within the Family Care Program for older adults and adults with disabilities.

Responsibilities
  • Perform telephonic and face-to-face functional assessments to identify, evaluate, coordinate, and manage member needs across physical health, behavioral health, social services, and long‑term care.
  • Manage members with chronic illnesses, co‑morbidities, and/or disabilities to ensure cost‑effective and efficient utilization of health benefits.
  • Obtain thorough and accurate member history to develop individualized care plans.
  • Establish short- and long-term goals with members, caregivers, family, natural supports, and physicians.
  • Develop and oversee care plans, ensuring member access to required services.
  • Assist with authorization/referral facilitation for service utilization within benefits structure or through extra‑contractual arrangements.
  • Interface with Medical Directors, Physician Advisors, and Inter‑Disciplinary Teams for person‑centered care plan development, and support problem‑solving with providers, claims, or service issues.
Minimum Requirements
  • Associate of Science in Nursing or Bachelor of Science in Nursing with a minimum of 3 years of experience in a Service Coordinator, Case Management, or comparable role with individuals with chronic illnesses, co‑morbidities, and/or disabilities.
  • Current, active, valid, and unrestricted RN license in Wisconsin.
  • At least one year of experience working with at least one of the Family Care target populations.
Preferred Skills, Capabilities & Experiences
  • BA/BS in Health/Nursing preferred.
  • Case management experience with older adults or individuals with disabilities.
  • Long-Term Care background preferred.
  • Commitment to member-centered care and cultural responsiveness.
  • Family Care/Partnership experience preferred.
  • Comfortable using technology.
  • Strong interpersonal and communication skills.
  • Ability to work collaboratively in a team environment.
  • Problem‑solving and crisis management abilities.
  • Willingness to travel to work sites and member locations for in-person visits.
