Position Title: Registered Nurse
Location: Upland Hills Health - Dodgeville Hospital Campus
Role & Department: Registered Nurse that will work in the Continuous Care Services Department in Palliative Care Services
Hours & Shift: Full-time (1.0 FTE) day shift position, Monday through Friday 8 a.m. to 4:30 p.m.
Role Responsibilities:
- The Palliative Care Registered Nurse works in collaboration with an interdisciplinary team to deliver palliative care for patients and families who are dealing with serious chronic illnesses
- The Palliative Care Registered Nurse will help educate patients and families with a focus on improving symptom management as well as increasing knowledge of medical options and how they impact quality of life
- Responsible for utilizing the professional nursing process to provide individualized nursing care to patients across the lifespan as characterized by the efficient, focused assessment of patients
- Present a proactive role with Providers, Nursing, Pharmacy, Continuous Care Nurses and other departments to provide palliative care services to patients and their families and implement guidelines that support the complex patients throughout the system
- Care for patients and families in all phases of preventative care, health maintenance, diagnosis, treatment and follow-up as patients’ transition through care settings
- Act as a patient and family advocate and assist the patient and family in developing skills so that they can advocate for themselves while preserving and protecting patient and family autonomy, dignity and rights
- Responsible for the coordination of patient care, screenings, education, triage and various other tasks within the RN scope of practice
- Collect, prioritize and synthesize available data to identify unique patient needs
- Develop and maintain care plans with patients in collaboration with the patient’s care team
- Assess cultural and age-specific considerations for appropriate care planning
- Accurately document all nursing care and related services in the patient health record
- Actively participate in ongoing quality improvement activities
- Provide for safe patient transitions and hand offs through effective communication with patients, family and care providers
- Ability to manage and prioritize multiple assignments and tasks while setting priorities and adapting to changing conditions
- Ability to work independently in an ever-changing environment while having the ability to share responsibilities with others
- Exceptional listener and communicator with ability to convey information clearly, concisely and logically both verbally and in writing
- Ability to relate to coworkers, solve problems, and mitigate problems with others
- Ability to deal with stressful situations for extended periods with good attention to detail
- Good analytical skills to assess specific needs of patients and adjust care, instruction or interaction with patient
- Be a resourceful team player with ability to build trusting relationships with patients and colleagues with the ability to get along well with other from diverse backgrounds
- Ability to maintain confidential information
- Associates or Bachelor Degree in Nursing
- Have a current and valid Registered Nurse License from the State of Wisconsin
- Hold a current Basic Life Support Certification (BLS)
- One to two years of experience in a clinic or hospital setting
- Palliative Care experience preferred
- Must have knowledge of motivational interviewing
- Chronic Care Management and Hospice background also preferred
- Prior experience using the EPIC medical record system is preferred but not required
- Previous experience with the Microsoft Office Suite software programs is beneficial
