Hours & Shift: Full-time (1.0 FTE) day shift position, Monday through Friday 8 a.m. to 4:30 p.m.

- The Palliative Care Nurse Practitioner conducts inpatient, outpatient, nursing home and home visits in Palliative Care.
- Conduct face-to-face hospice certification visits in the home setting as needed.
- Provide professional services to patients of all ages in accordance with Upland Hills Health’s Palliative Care and Hospice philosophy and standards.
- Collaborate with primary care providers to establish and maintain appropriate care to meet patient needs.
- Provide face-to-face and clinical visits with hospice patients as needed.
- Assess hospice patients for hospice recertification as needed.
- Maintain records, charting and generate reports as necessary.
- Manage pain and other distressing physical symptoms in a timely manner.
- Understand and communicate patient/family goals of care changes.
- Provide end-of-life education to patients and families with compassion and respect.
- Support patients approaching last days of life and assist family members through the process.
- Assess psychosocial and spiritual needs appropriately.
- Report findings to the primary care provider, continuous care services director and other appropriate medical providers.
- Attend in-services and training sessions to maintain credentials and stay up to date.
- Assist with monitoring of Performance Improvement activities to achieve and maintain desired outcomes.
- Comply with all applicable rules and regulations, including current Palliative Care guidelines, HIPAA, Hospice Conditions of Participation and Upland Hills Health policies.
- Attend hospice interdisciplinary meetings as needed.
- Complete other duties to provide excellent patient care while meeting departmental and organizational goals.
- Exhibit exceptional listening and communication skills for clear, concise verbal and written information.
- Demonstrate good analytical skills to assess patient needs and adjust care, instruction, or interactions.
- Maintain confidential information.
- Master's Degree in Nursing
- Current and valid Registered Nurse license and licensed as an Advanced Practice Nurse Prescriber (APNP) from the State of Wisconsin
- Current DEA number
- Board certified
- 3-5 years experience in Family Practice setting as a Nurse Practitioner
- Palliative Care experience highly preferred
- Chronic Care Management and Hospice background also preferred
- Prior experience using the EPIC medical record system is preferred but not required
- Experience with Microsoft Office Suite is beneficial
