Registered Nurse; RN - Nursing & Rehab Center - p.m. shift

Registered Nurse – Full-Time/Part-Time

Location: Upland Hills Health – Dodgeville Hospital Campus, Upland Hills Health (UHH) –
Role: Registered Nurse in the Nursing & Rehab Department –
Hours & Shift: Part-time to Full-time (0.5–1.0 FTE, 20–40 hours per week) –
Shift: p.m. – Limited weekends and a holiday rotation.

• Provide high quality, resident‑centered care for all residents with dignity, respect, confidentiality, and resident rights.
• Serve as a direct care provider and oversee other nursing staff services as required.
• Collaborate with physicians, nursing staff, and ancillary professionals to deliver high quality patient care.

• Associate’s Degree in Nursing or Bachelor’s Degree from an accredited institution.
• Licensed Registered Nurse in the State of Wisconsin or completing licensing soon.
• CPR certification or completed within 90 days of employment.
• Prior nursing services experience preferred; not required.
• Prior long‑term care experience very beneficial.
• Prior experience using Point Click Care beneficial; not required.

This position is eligible for “up to a $5,000.00 Employment Bonus” prorated based on the FTE. Bonus payments will be made based on the FTE schedule below:

• 1.0 FTE – $5,000
• 0.9 FTE – $4,500
• 0.8 FTE – $4,000
• 0.7 FTE – $3,500
• 0.6 FTE – $3,000
• 0.5 FTE – $2,500

The Employment Bonus will be paid out at set times during the first 3 years of employment in 6 separate equal installments. Eligible nursing positions hired at a 0.5 FTE status or greater qualify; candidates must continue to be employed at a 0.5 FTE status or greater at each bonus payment to receive it.
