Job Description:

Provide wellness visits, in-home assessments, and comprehensive geriatric and adult physical exams for the elderly and adults.

Pay: $100 per assessment with mileage reimbursement. Expect $2,000–$10,000 per month based on 20–100 completed visits.

• Conduct adult/geriatric health risk assessments, including medical history, diagnosis, treatment, health education, and physician referrals.
• Collaborate with physicians, case managers, social workers, family members, caregivers, and ancillary medical personnel.
• Provide services in home visits and skilled facility visits with flexible scheduling (days, evenings, nights, weekends).
• Order and interpret diagnostic imaging and laboratory results; administer point-of-care and lab-based testing.
• Document patient encounters to clinical standards and protocols, ensuring compliance with PHI, OSHA, and nursing board policies.
• Act as a preceptor for new providers and perform all mandatory compliance training.

• Bachelor of Science in Nursing preferred;
  Master’s degree required or commensurate experience.
• Current RN and NP licensure in Texas with prescription authority or eligibility for prescriptive authority.
• Board certification by AANP, AACN, or ANCC in Adult, Family Practice, Geriatrics, or Adult-Gerontology Primary Care; acute‑care NP certifications accepted.
• Current CPR certification and multistate licensure preferred.
• Valid state driver license with proof of coverage and compliance with company liability insurance standards.
• Strong verbal and written communication skills, proficient computer skills, and strong listening and empathy.
• Ability to travel to assigned locations via public transportation, including airplanes.
• Proficient in Microsoft Office and email.
