Specialty: Geriatric Care - NURSING HOME - Advanced Practice Provider

Schedule: 1.0 FTE Monday-Friday between 6:30am and 5:00pm. Travels to assigned nursing home facilities, covering the Fox Valley from Appleton to Oshkosh (includes Kiel and Chilton).

Practice Detail: Family Medicine practice in a nursing home environment. Provides onsite care to nursing home patients at various locations; works with a support team onsite to assist with patient care. Experience preferred. Visit 12+ patients per day in the skilled nursing facility. Responsible for schedule to assigned sites, or as needed.

Position: Ascension-Wisconsin is seeking a Board Eligible/Board Certified Advanced Practice Provider (Nurse Practitioner/Physician Assistant) to join our Nursing Home Team. This is a great opportunity to provide medical services to patients in the Fox Valley.

• Call Schedule is 1:6, with no nights
• Busy schedule with travel within the Fox Valley; experienced practitioner preferred in Family Medicine

• In collaboration with a physician, diagnose and treat certain acute or chronic health conditions to expand individualized primary healthcare services.
• Obtain patient history, perform assessments, order/perform/interpret diagnostic studies, and conduct preventative health assessments, screenings, immunizations.
• Provide direct treatment and management of health conditions or referrals as needed.
• Counsel and educate patients and families/caregivers on preventative health, treatment options, and community resources.
• Document delivery of health care and nursing processes in accordance with standards and policy.

• Licensure / Certification / Registration: BLS Provider within 30 days of hire;
  Nurse Practitioner credentialed from the Wisconsin Board of Nursing;
  Registered Nurse credentialed from the Wisconsin Board of Nursing; DEA credentialed (preferred, verified by Credentialing Office).

• Education: Master’s degree in Nursing required.
