Physical Therapist

We are seeking a PRN (pro re nata) Physical Therapist to fill a recent opening. This position provides skilled therapy services for our Inpatient, Outpatient and Home Health Programs, and is supported by a team of Occupational, Physical and Speech therapists. PRN hours are encouraged. Ingleside Manor offers an in-house therapy program with updated equipment and private treatment areas. Our EMR system (Optima Health Solutions) allows documentation, billing and esignature in one location.

Ingleside Manor is located in Mount Horeb, WI, just south of Madison, and has provided quality care to seniors for more than 45 years.

• Supervise Physical Therapy Assistants, temporary licensees, students and support personnel in accordance with state licensure requirements and professional standards (as necessary).
• Meet patient care hours (PCH) requirements for the rehab gym, or as directed by the Program Manager.
• Attend and contribute to patient care, staffing conferences and related meetings.
• Refer to other disciplines and agencies to address the comprehensive needs of the patient.
• Collaborate with other health care team members to select appropriate therapeutic regimens aligned with patient needs and capabilities.
• Adapt the program and/or plan of care as needed for individual patients.
• Promote a safe environment and enforce the elimination of fire and safety hazards.
• Order supplies and equipment as necessary with approval from the Program Manager.
• Maintain and report on equipment condition and required repairs.
• Put patient service first—deliver high-quality, compassionate care that respects patient and family needs and rights.
• Screen, examine and evaluate patients, including history, systems review, tests and measures, and develop diagnosis and plan of care; establish treatment goals and document all related services.
• Provide therapies including therapeutic exercise, gait and locomotion training, neuromotor development, manual therapy, and other modalities as appropriate.
• Design, fit and train patients in adaptive devices, orthotics and prosthetics when applicable.
• Instruct patients in mobility skills and monitor progress and rehabilitative potential.
• Perform routine reexamination as needed to modify the plan of care.

• Valid Physical Therapy license in Wisconsin or proof of license eligibility.
• Master’s or Doctorate degree in Physical Therapy or a related field.
• Alternative credential acceptance per state licensing rules (e.g., pre-2002 criteria) as outlined by the job requirements.
• Foreign-degree equivalency accepted as per state licensing rules.
• Excellent communication skills and strong organizational/time-management abilities.

Pay Range: $44-$50/hr depending on experience.
