We’re hiring for a Part-Time (20–30 hrs/week) or Full-Time (30+ hrs/week) Physical Therapist to support our growing team in Southwest Wisconsin.

Your RoleAs a Home Health Physical Therapist, you’ll deliver skilled therapy services based on individualized care plans and physician orders. You’ll work closely with the interdisciplinary team to support patients in regaining strength, mobility, and independence at home.
Key Responsibilities- Complete OASIS assessments, evaluations, and progress notes
- Develop and revise care plans
- Perform skilled physical therapy procedures
- Educate patients and families on home programs and daily activities
- Collaborate with physicians and the care team
- Participate in IDT meetings, in-service training, and QA initiatives
- Ensure accurate documentation and use of ICD-10 codes
- Active Wisconsin Physical Therapist license
- CPR certification
- Valid WI driver’s license & car insurance
- Two (2) years of experience preferred
- Comfortable working independently in a home care environment
- Hiring manager will contact you within 1-2 business days of your application submission
- Complete 1-2 interview(s) and meet with immediate supervisor and mentor
- Upon receiving the job offer, review job description, and complete new-hire tasks and onboarding paperwork (via the Paycor app)
