Physical Therapist - PRN – Ingleside Communities
We seek a skilled Physical Therapist to provide therapy services across Inpatient, Outpatient, and Home Health programs at Ingleside Manor, a long‑standing elder care facility in Mount Horeb, WI. As a PRN (pro re nata) provider, you will collaborate with Occupational, Physical, and Speech therapists to deliver high‑quality, patient‑centered care seven days a week. The role requires proficiency with Optima Health Solutions EMR for documentation, billing, and electronic signatures.

- Supervise Physical Therapy Assistants, temporary licensees, students, and support personnel per state and professional standards.
- Meet patient care hours (PCH) requirements for the rehab gym as directed by the Program Manager.
- Participate in patient care conferences and related meetings.
- Make referrals to other disciplines or agencies to address comprehensive patient needs.
- Consult and collaborate with the healthcare team to select appropriate therapeutic regimens.
- Adapt program and/or plan of care based on individual patient needs.
- Promote a safe environment and eliminate fire and safety hazards.
- Order supplies and equipment upon Program Manager approval.
- Maintain equipment; identify and report repairs.
- Provide patient‑first service ensuring quality, compassionate care.
- Screen, examine, evaluate patients; develop diagnoses, goals, and plans; document all related services.
- Apply therapeutic modalities, exercise, gait training, manual therapy, airway clearance, and other interventions as needed.
- Design, fit, and train patients in adaptive devices such as orthotics and prosthetics.
- Select activities suitable for patients’ physical and mental capacities.
- Instruct patients in mobility skill techniques; evaluate progress and behaviors.
- Perform routine re‑examinations to modify or progress treatment plans.
- Valid Physical Therapy license in Wisconsin or proof of license eligibility.
- Master’s or Doctorate degree in Physical Therapy or related field.
- For those with pre‑2002 PT education: a Bachelor’s degree and five years of progressively responsible PT experience may substitute the graduate degree.
- Foreign‑degree equivalent to the first professional degree may substitute the Master’s.
