Transitions At Home, a non‑profit home health organization serving Southern Wisconsin, is currently seeking to hire a Full‑Time Physical Therapist.
We’re seeking a compassionate and driven Physical Therapist to provide personalized, one‑on‑one care in patients’ homes—helping them regain strength, mobility, and independence.

What a Transitions at Home Physical Therapist doesAn Physical Therapist (PT) administers physical therapy to patients on an intermittent basis in their place of residence in accordance with physician orders and a plan of care under the direction and supervision of the Director of Clinical Services / Clinical Manager.
Hours- 8:00am - 4:30pm
- Monday through Friday
- No holidays

Qualifications /Job Requirements
- Active Wisconsin Physical Therapist License
- Two (2) years’ experience preferred
- CPR Certification
- A valid Wisconsin Driver’s License and Car Insurance
- Hiring manager will contact you within 1-2 business days of your application submission
- Complete 1-2 interview(s) and meet with immediate supervisor and mentor
- Upon receiving the job offer, review job description, and complete new‑hire tasks and onboarding paperwork (via the Paycor app)
