Location: Mount Horeb

Ingleside Communities, located in beautiful Mt. Horeb, WI, is currently hiring for a full-time Care Coordinator to work in our CBRF.

Overall responsibility of managing the RCAC and CBRF on the Ingleside Campus, ensuring that the assisted living facility is compliant with Federal and State Regulations, and the overall wellbeing of the residents, families, and staff. Assisting the Assistant Administrator with the day-to-day operations of the facility.

Training and Experience
  • Must have at least 3-5 years experience working as a C.N.A. / R.A., prior management experience is a plus, but we are willing to train the right candidate.
  • Have been trained in fire safety, first aid, Block I, and or C.N.A. certified and the basic skills set needed in order to perform the job of a Care Coordinator.
Essential

Job Duties and Responsibilities
  • 1. Attend stand-up meeting every morning at 9:15 AM
    • Will go over resident grievances/issues
    • Staff-related issues
    • What needs to be done today
    • All other items that need to be addressed
  • 2. Work alongside the Administrator to ensure that the facilities are running properly and assist as needed
    • Will meet daily/weekly to discuss any issues that arise
  • 3. Address and resolve any staffing issues, involving the Assistant Administrator as needed
    • Handle employee complaints or inadequate behaviors
    • Issue write-ups or implement PIPs as appropriate
  • 4. Enforce safety issues at both facilities
  • 5. Review and implement corporate policies and procedures
  • 6. Build schedule and post in Paycor system for employees
  • 7. Maintain a safe and secure working/living environment by monitoring staff, building maintenance, and housekeeping
  • 8. Prepare and post the monthly schedule by the 10th of the month prior
    • Work to fill any gaps
  • 9. Must be able to report to work for any of the following issues
    • Allegations of abuse to a resident
    • Emergency issues
    • Staff shortages
  • 10. Investigate all injuries to staff, family, or residents
    • Send in self-reports as needed
    • Update Assistant Administrator on all injuries
    • Report injuries to Workman’s comp – work with HR
  • 11. Ensure staff is doing their continued education in healthcare academy
    • Print out certificates and sign the continuing education book
  • 12. Set up monthly staff meeting to discuss issues, quality improvement initiatives, and to promote yearly training that needs to be completed
  • 13. Ensure that all resident Individual Service Plans are completed and signed by staff, family, assistant administrator, and resident
  • 14. Attend and facilitate care conferences
  • 15. Work with Assistant Administrator on annual and probationary performance evaluations
  • 16. Work with admissions to admit any new residents into the facility
  • 17. Other job duties/ responsibilities as assigned by the Assistant Administrator
  • 18. Responsible for hiring, retention, and recruitment of staff
On-Call
  • 1. Care Coordinator will be on-call regularly
  • 2. Care Coordinator and Assistant Administrator will work with each other for being on-call when the other is on PTO.
  • 3. Inform the Assistant Administrator of any building/resident concerns if needed.
Scheduling/Timecards/Paycor
  • 1. Post the schedule by the 10th of the month with all open shifts available
    • If open shifts are not picked up by the 15th mandate 12 hours shifts
    • Put schedule into Paycor schedule by the 20th of the month
    • Check Paycor daily for:
      • Absentee employees with person who worked
      • Employee swaps
      • Mandating if needed
      • When making changes place a note of why the change
  • 2. When changes need to be made to the schedule for the next few days up to a week discuss with all employees and those you are making changes to as well as for the following week and discuss the changes, why the changes are needed, if they are agreeable, and let them know that the rest of the schedule is changing and that they will need to look at it.
  • 3. Check Timecards on the following days to ensure that staff is punching in and out correctly
    • At the end of each week
    • Put in on-call bonus
    • Ensure that everyone who has missed punches has filled out a form remind them if they don’t
    • On the 16th and the last day of the month check again to ensure that timecards are good for payroll
  • 4. Keep up on all Attendance records and disciplinary actions…
