Aspirus Peach Street Clinic in Wisconsin Rapids, WI is seeking an RN‑Ambulatory Care Coordinator to join our team!
The Ambulatory Care Coordinator is a Registered Nurse (RN) directly responsible for assessing, coordinating, and integrating healthcare for patients with chronic conditions in the Aspirus Clinic System. This RN position manages all aspects of patient‑centric care coordination through collaboration with physicians, staff, and other health professionals to provide services across the health care continuum for all patients within the clinic setting.
The Ambulatory Care Coordinator will work directly with the entire health care team to deliver innovative population health management practices to the patients and community of Aspirus.
HOURS:
Full‑Time or 1.0 FTE, 80 hours every pay period. Day shift.
- Knowledge of patient care standards and practices is normally acquired through completion of a nursing program.
- Two years’ experience in department/specialty necessary to enhance and develop skills, with previous experience in supervisory management, leadership, or teaching capacity beneficial.
- Possesses an active Wisconsin RN nursing license.
- Current BLS certification – all clinical personnel in identified job codes are required to maintain Basic Life Support (BLS) training through independent study, online manikin, and knowledge testing. An acceptable training program is the Basic Life Support (BLS) Health Care Provider course from the American Heart Association or the American Red Cross Professional Rescuer Program with AED (see Cardiopulmonary Resuscitation Training Policy).
- Advanced Cardiac Life Support (ACLS) for adult patient units and NRP for NICU.
- BSN required, or actively working towards degree to be obtained within 4 years of the hire date in that role.
- Additional specialty certifications as specified by department protocol (i.e. PALS, TNCC, etc.).
- Certification for area of specialty required or required to pursue certification in professional development within three to five years of eligibility.
- Master’s in nursing preferred.
- Member of professional nursing organization required (i.e. ENA, CCRN, AACN, etc.).
