Comforting, Loving & Helping Hands Home Care is seeking an experienced Licensed Practical Nurse (LPN)in Winter Park, FL to provide skilled nursing services to patients in their homes. This role includes routine skilled nursing visits and may include assessment‑related visits under RN direction

This position is ideal for LPNs who are confident working independently, experienced in home health, and understand the importance of accurate, detailed clinical documentation in supporting patient care and nursing oversight.
Who This Role Is Best ForThis role is well‑suited for LPNs who:
- Have a minimum of two (2) years of nursing experience
- Have experience in home health or community‑based nursing
- Are comfortable completing skilled nursing visits under established care plans
- Understand the role of documentation in supporting RN supervision and care planning

One‑on‑One Patient Care
Provide focused, individualized care in patient homes without the pace of facility‑based settings.
Flexible Compensation Structure
- Compensation may be per visit or hourly
, depending on the case - Assessment‑related visits and routine visits are compensated differently based on scope and responsibility
Flexible Scheduling
- PRN, per‑diem, or flexible caseloads
- Visit schedules coordinated based on nurse availability and patient needs
- Perform routine skilled nursing visits as ordered
- Assist with patient assessments under RN direction, as applicable
- Administer medications and treatments within LPN scope of practice
- Provide wound care, catheter care, and other skilled interventions
- Monitor patient condition and report changes to the supervising RN
- Educate patients and caregivers on care plans and treatment instructions
- Complete accurate, timely, and defensible clinical documentation
LPN documentation is a critical part of patient care and is used to:
- Support RN oversight and clinical decision‑making
- Contribute to the development and revision of the patient’s Plan of Care
- Meet payer and regulatory documentation requirements
Strong attention to detail and timely completion of visit notes is required.
Requirements- License: Active Florida Licensed Practical Nurse (LPN) license
