The Physical Therapist is responsible for providing medically prescribed, goal-oriented therapy in accordance with the Physician Plan of Care, company policies, and professional standards of practice. You'll collaborate with an interdisciplinary team to help patients live their best possible lives.

• Develop individualized patient plans of care and communicate with physicians as needed.
• Provide physical therapy visits and document care accurately and timely.
• Educate patients and families to support independence and health.
• Participate in interdisciplinary team meetings to coordinate care and exchange updates.
• Ensure compliance with agency policies,
  federal/state regulations
  , and Joint Commission standards.
• Complete all required documentation in an accurate and timely manner.

• Current Physical Therapist (PT) license in Florida in good standing
• Current CPR certification
• Level 2 background screening
• Recent negative TB or chest X‑ray
• Knowledge of physical therapy principles, methods, and practices
• Strong written/verbal communication and collaboration skills
• Ability to build rapport with patients and families
• 1+ year of PT experience in a multidisciplinary setting preferred
• Kinnser / Well Sky EMR experience preferred
