APP - Primary Care Network, Winter Haven

Overview

Advent Health Primary Care Network is seeking an Advanced Practice Provider to join our team. This model focuses on delivering comprehensive care that includes preventive care, chronic disease management, and acute care. We place a strong emphasis on patient-centered care within the network through patient engagement, digital health tools, cultural sensitivity, and accessible locations.

• Flexible 4- or 5-day workweek with no weekend office hours
• Minimal call responsibilities with shared holiday rotation

• Malpractice coverage for peace of mind, including tail coverage

• Provide comprehensive, patient-centered primary care to pediatric through geriatric patients, including women’s health
• Deliver independent and collaborative medical management of acute and chronic conditions
• Order and interpret diagnostic testing, including labs and imaging
• Monitor treatment plans and coordinate referrals to physicians or specialists as needed
• Collaborate closely with physicians and clinical staff to ensure continuity and consistency of care
• Deliver care with a professional, compassionate, and patient-focused approach

• Master’s degree in nursing (FNP/APRN) or Physician Assistant Studies
• Minimum of 1-2 years of clinical experience in Family or Internal Medicine preferred
• Experience caring for patients of all ages and managing acute and chronic conditions strongly preferred

• Active Florida APRN or PA-C license
• Active DEA license
• ANCC, AANP, or NCCPA (or equivalent) certification
• BLS required; ACLS (preferred)
