Job Details

* 15 patients per day

* Part-time position: 1-3 days a week

* Compensation and benefits: our compensation is fee for service (FFS); expected compensation is estimated salary range of $82,252 to $98,703 annually with no cap on productivity income potential.

Key Responsibilities

* Perform primary care in a post-acute setting for short term and long-term patients

* Manage chronic disease, diagnose illnesses and medical conditions, and implement plan of care

* Order and interpret lab results

* Prescribe and monitor medications

* Consult with physicians and coordinate patient care with other healthcare practitioners

* Educate patients and their caregivers on coping skills to handle chronic and age-related conditions

* Build relationships with your patients, patients' family members, and facility staff to facilitate effective treatment delivery

* Promote healthy lifestyle changes

Qualifications

* Board certified internal medicine (IM), family medicine (FM), or emergency medicine (EM) clinician

* Willing to train experienced candidates as well

* Licensed in state
