ResponsibilitiesAs a RN in Home Health, you’ll deliver patient-centered care in home settings, collaborating with physicians, therapists, and caregivers to create comprehensive care plans. Core responsibilities include:
- Conducting thorough assessments and monitoring vital signs
- Administering medications per orders
- Managing wounds and IV therapies as prescribed
- Implementing evidence-based interventions to prevent rehospitalization
