Position: Transitional Housing Case Worker
POSITION TYPE FULL-TIME, 37.5 HOURS/WEEK
Position Summary: This is a new role being introduced. The Transitional Housing Case Worker will support participants of all backgrounds who are living in our transitional housing for women, gender diverse folks, and children who are exiting situations of violence. They will support people on their healing journey, through one-on-one support, case coordination, program delivery, system navigation, advocacy and referrals to relevant resources.
Responsibilities:
To provide individual support and ongoing case management to people who have been impacted by violence to set goals and create plans to reach those goalsTo empower people to advocate for themselves and to advocate on their behalfTo assist people with finding permanent housing and connecting them to income supportsTo provide information, referrals and system navigation to participants and their familiesRegular follow-up and checks-ins with transitional housing tenants, including wellness checksSupport participants to prepare for external appointments and accompany participants to offer support and companionship as neededTo work in collaboration with internal and external programs to ensure effective wrap around supports for participantsMaintain positive working relationships with service providers, and advocate to them on behalf of participants as neededReport on program outcomes to the Director of Community Development as required by funding agreementsKnowledge, Skills and Abilities:
Essential
Experience supporting victims of gender-based violence and knowledge of available resourcesExcellent advocacy skills as well as knowledge and experience with EIA and housing systemsSolid grounding in harm reduction, trauma-informed, and empowerment-based approachesDemonstrated ability to work within anti-racist and anti-oppressive frameworks and actively engage in ongoing learning about these frameworksCase management experience, including maintaining case notes and reportingExcellent interpersonal skills and the ability to build trust and rapport with clientsProfessional, responsible, and discreet when handling sensitive and confidential informationOrganizational and time management skills with the ability to manage a dynamic caseloadStrong written and verbal communication skillsProficiency in Microsoft Office and OutlookStrong ability to work collaboratively in a diverse staff teamSelf-motivated and able to work with minimal supervisionStrong attention to detail and follow-upDriver’s license and access to a vehicle requiredPreferred
Post-secondary degree in the Social Services fieldExperience working within the non-profit sectorAbility to work from a multi-level approachWorking conditions:
Days, with evening and weekend hours required as neededFrequent case management related travel within the communityThere is a risk of exposure to potentially unpredictable behaviors and situations
