About the CompanyMain Street Project (MSP) is a community health centre with a mandate to address the social determinants of health which give rise to issues of addiction, physical and mental health issues and homelessness. MSP uses housing‑first and harm‑reduction principles in the provision of a safe, respectful and accessible place for individuals who are experiencing homelessness or at risk of homelessness in the community.
Since 1972, MSP has been providing a range of services to Winnipeg’s most marginalized residents, including emergency shelter and nutrition services, drug and alcohol withdrawal management services, health advocacy and support, case management services, housing support services, food bank services and a mobile community outreach programme. Main Street Project is committed to developing impactful and meaningful solutions that drive positive changes in our community.
The case management team is a key connection to outside organizations and participants accessing MSP services. Team members identify, develop, implement and assess participants’ self‑identified goals using a housing‑first, harm‑reduction, trauma‑informed lens. Through relationship building and coordination with primary care, income, addictions and mental health supports, case managers help participants secure and maintain housing.
Key Responsibilities- Provide ongoing case management support to assess progress and ensure treatment plan.
- Develop and coordinate individualized care plans addressing short‑term and long‑term goals in collaboration with participants, interdisciplinary team members, and external partners.
- Assess an unsheltered person’s needs and proactively identify barriers to housing, developing strategies to help participants meet goals.
- Create and maintain professional relationships with community members, including boundary setting.
- Keep accurate, continuous, legible and signed records including progress notes, checklists, correspondence, and journals for individual and collective access in accordance with established protocols for casework files.
- Work with involuntary participants and participants who cannot give consent.
- Apply case‑management ethics to all participant relationships, including confidentiality, self‑determination, informed consent and the power differential.
- Plan interventions with participants.
- Identify and provide information and resources to participants:
- Community resources
- Rent and income assistance
- Mental health resources
- Indigenous organisations
- Establish partnerships with landlords and service agencies, advocating on behalf of participants.
- Identify symptoms of suicide ideation and offer resources and support.
- Create environments of cultural safety for participants.
- Meet with participants both on‑site and in the community.
- Monitor, evaluate and communicate progress of participants with themselves, team members and managers regularly.
- Maintain accurate, confidential electronic and file records, including statistical data.
- Conduct crisis intervention as needed.
- Attend program‑related community, coalition and committee meetings.
- Outreach:
- Build effective relationships with encampment residents.
- Coordinate with local agencies to provide services that meet residents’ needs.
- Contribute to the development of a community that values cultural diversity.
- Preferred experience with homeless outreach, proven ability to access a wide range of homeless services, familiarity with overcoming barriers encountered by homeless people.

- Provide supportive and proactive approach to eviction prevention.
- Create an open and welcoming environment for tenants to share housing concerns.
- Review client risks or issues and utilize a team approach to identify an action plan.
- Experience with palliative and after‑death care.
- Experience with harm‑reduction in a supportive living environment.
