Position OverviewThe Case Coordinator will determine program eligibility and assess the need for care at home or in a personal care home for the elderly or infirm adult. They will coordinate the delivery of a broad range of professional and non-professional services based on the Case Coordinator Guidelines and act as a liaison with agencies and facilities involved with the client group.
Responsibilities- Receive referrals and assess client and family to determine home care needs.
- Assess and prioritize patterns/needs; establish an appropriate discipline coordinator.
- Develop a plan of care for the Continuing Care Program, including statements of client need, objectives, service provision and evaluation criteria.
- Implement and coordinate the care plan, including initiating medical rehabilitation and consultative services as needed.
- Provide professional intervention through counseling, teaching, guidance, or crisis intervention where appropriate.
- Maintain current case counts and ensure proper submission of statistics.
- Plan and organize work schedule and manage caseload demands; utilize consultation and supervision.
- Gather data on resources and resource needs related to caseload/community.
- Interpret the program and resources to the public and other agencies.
- Establish and maintain liaison with local health care services and the informal community resource network.
- Participate in educational development for new staff/students related to the program.
- Keep current developments within the discipline as they relate to the Continuing Care Program.
- Two years of directly related community experience preferred.
- Previous experience as a Case Coordinator in Community Health Services preferred.
- Experience with EFT implementation considered an asset.
- BSW, OT, PT, BSc PN, RPN, BN, RN, SLP, RD or related health/human service degree required.
- Valid driver’s license and a vehicle.
- Licensure/registration as per professional designation required.
- Computer literacy; effective oral and written communication skills.

Additional InformationThis position requires a current satisfactory Criminal Records Check (including Vulnerable Sector Search), Child Abuse Registry Check and Adult Abuse Registry Check as conditions of employment. A security check is current if obtained no more than six (6) months prior to start. WRHA policy allows only up to 1.0 EFT across the WRHA entities.
