Position: ADULT FAMILY SUPPORTS - Case Manager (Term)

New Directions is a non-profit social service agency offering a wide range of resources and services that provide supports in a variety of settings and foster people’s hopes and dreams in their communities.

Adult Family Supports (AFS) is one of the many services offered by New Directions and provides services for adults who may have intellectual disabilities and/or mental health issues living in various settings including family homes, independently, and transitional support.

Responsibilities of the Case Manager include:
  • Foster positive, respectful, and non-judgmental relationships with individuals and their supports.
  • Build relationships with the person receiving support, their families, and networks of support, and establishing or maintaining connections with culture of origin.
  • Development and implementation of support plans in collaboration with individuals supported and stakeholders.
  • Collaborate with individuals and their networks of support to coordinate services, including planning, navigating community resources, making referrals, and advocating for services.
  • Maintain standards for administration and documentation, aligned with best practice and appropriate risk management.
  • Liaise with funders, families, and other community support services.
  • Provide effective financial management specific to the needs of each individual.
  • Hold knowledge of addictions supports, services and resources available in the community.
  • Direct and delegate tasks to Community Resource Workers to develop and meet the needs and goals of individuals receiving services.
  • Participate in the On-Call rotation.
  • Other assigned duties as required.
Applicants must have
  • B.S.W./B.A. or related social services education such as Family Therapy, Human Ecology, and Indigenous Studies
  • Minimum of 5 years’ experience working with adults with a label of intellectual/developmental disability and with adults in a community setting
  • Demonstrated knowledge of person led practices, knowledge of Deaf and Indigenous Cultures
  • Ability to use the Microsoft Office suite of products, including Word, Excel, Outlook, and Share Point
  • Excellent communication skills including mediating difficult conversations and conflict resolution
  • Critical thinking and problem-solving skills
  • Excellent time management and organizational skills
  • A valid Class 5 driver’s license and access to a reliable vehicle for work purposes
Other Qualifications considered an asset include:
  • Fluency in American Sign Language
  • First Aid Certification
  • Nonviolent Crisis Intervention training
  • Experience supervising staff.

The salary range for this full-time, 1-year term 70-hour bi-weekly position is $26.28 - $33.90 per hour (equivalent to $47,829 - $61,698 per annum), dependent upon education and/or experience.

The successful candidate will be subject to Criminal Record Check, Adult and Child Abuse Registry Check, Prior Contact Checks; and must supply a current driver’s abstract.
