Position Summary

Functioning as part of the coordinated service delivery team, the Outreach Worker works proactively with new and existing clients, offering a range of support and recovery services, within an existing service plan and on an as‑needed basis. The Outreach Worker is a relationship‑builder and advocate and provides services in a non‑judgmental and culturally appropriate manner, helping clients to build new skills and achieve their goals.

This Program works within an Assertive Community Treatment Model and utilizes the Housing First Principles from a social justice orientation which includes harm reduction, pro‑choice, social determinants of health, human rights, social development, population health, health equality and trauma informed, client centered practice.

Qualifications
  • Education: Valid driver’s license and access to a reliable vehicle
  • Skills, Expertise and

    Experience:

    Minimum of 2 years’ experience working in community, preferably with program target population; experience providing outreach with hard‑to‑reach population
  • Critical

    Skills:

    Proven knowledge and ability in relationship‑building, critical thinking, facilitation, decision‑making, problem‑solving and conflict resolution skills
  • Focus on Results: Demonstrated ability to prioritize and focus work‑related activities to effectively and efficiently achieve service goals and produce expected results/outcomes
  • Alignment with Organizational Values: Commitment to the principles that guide Mount Carmel Clinic, as reflected in the history, vision, mission and values
  • Commitment to Reconciliation: Knowledge and historical factors that affect the health and well‑being of Indigenous individuals and families
  • Teamwork: Proven ability to work within a team environment with respect, professionalism, cooperation, sensitivity and cultural awareness
  • MCC Philosophies: Strong knowledge, commitment and demonstrated ability to apply diversity, equity, cultural safety, pro‑choice, social determinants of health, population health, harm reduction, trauma‑informed and community development principles in all aspects of the work
  • Communication: Excellent verbal and written communication skills and proficiency in the use of computer systems
  • Assets: Post‑secondary education in a Social Services field; applicants with lived or living experience in the sex trade work are encouraged to self‑declare on their resume
