Discharge Planner-LPN – Full Time at Valley Health

This role assists RN Case Managers in creating comprehensive discharge plans, performing clinical assessments for patients requiring post‑acute Medicaid services and completing prenumeric duties such as pre‑screens, admissions, and insurance authorizations for Valley Health’s skilled nursing facility and transitional care beds.

• Assist Case Management staff with Level 2 (MIMR) assessment completion.
• Interview patients, family members, and caretakers to gather information necessary for assessments.
• Maintain open communication with the care management team concerning approval or denial of clinical assessments.
• Maintain proficiency in electronic submission programs and stay informed of current Medicaid clinical assessment policies and trends.
• Coordinate and follow up with community resources to facilitate seamless transitions for patients moving to igre
• Support patient movement to subsequent levels of care within the continuum of care.

Graduate of an accredited nursing school. Minimum two‑year bedside nursing experience.

• Licensed Practical Nurse (LPN) in the State of Virginia.
• BLS Certification (Basic Life Support) – American Heart Association approved. New hires must hold appropriate AHA certification prior to orientation.

Competence in interacting with patients, adapting to their special needs based on age and condition.
