As a compassionate Registered Nurse (RN) who finds purpose in supporting patients and families through meaningful moments of care, Legacy Hospice invites you to join our dedicated nursing team as a Full Time RN Case Manager (RNCM). We provide exceptional end‑of‑life care throughout the community.

Schedule
:
On‑Call coverage is required (one week every three weeks excluding most weekends and holiday on‑call coverage).

Territory
:
Winchester and surrounding counties.

Key Responsibilities
:

• Respond to clinical referral information with care and expertise, addressing inquiries and requests.
• Partner closely with the Clinical Director to assess eligibility and suitability of clients for hospice services.
• Provide direct nursing care, ensuring patient comfort and quality of life.
• Deliver top‑notch care rooted in proven principles, easing pain and bringing solace.
• Educate and advocate for patients and families, encouraging active participation in care planning.
• Participate in call duties as part of the team’s commitment to being present when it matters most.

Qualifications
:

• Current, unencumbered Registered Nurse license.
• Minimum one year of experience as an RN in a medical‑surgical or acute care setting; hospice experience a plus.
• Computer proficiency, including background in EMR.
• CPR certification if required by state.
• Valid driver’s license, vehicle insurance, and dependable transportation.

Salary
: $80,000 – $90,000 per year (average pay range). Final compensation will be determined based on experience and qualifications.
