In this pivotal role, you’ll:

  • Serve as the vital connection between hospitals and hospice, ensuring patients and families experience a smooth, dignified transition to end-of-life care.
  • Provide education, guidance, and advocacy that empowers families to make informed decisions.
  • Work closely with hospital staff, healthcare providers, and our hospice team to ensure seamless communication and continuity of care.
  • Make a direct and meaningful impact on quality of life by helping patients access timely, compassionate care when it’s needed most.
Base pay

$75,000.00/yr - $95,000.00/yr

Schedule & Location
  • Area:
    Winchester, VA
  • 32–40 hours per week
  • Includes some weekend coverage and flexibility for hospital program support
Position Summary

The Hospital Liaison provides a consistent presence at our community hospitals to establish and foster professional relationships with staff and physicians. The Hospital Liaison engages in collaboration with multiple disciplines including Social Workers, Discharge Planners, Case Managers, Nurses, Physicians, Nurse Practitioners, and Palliative Medicine. The Hospital Liaison coordinates referrals of new patients to Blue Ridge Care and ensures patients and families are educated regarding Blue Ridge Care services and philosophies.

Essential Duties & Responsibilities
  • Documentation & Communication: Ensure accurate and timely documentation of care provided and the patient’s/family’s response, adhering to organizational policies.
  • Facilitate continuity of service across care settings through effective information management, including both verbal and electronic communication.
  • Provide accurate clinical documentation in both the Blue Ridge Care EMR and hospital EMR.
  • Collaboration & Coordination: Collaborate with hospital staff regarding potential patient referrals to Blue Ridge Care.
  • Coordinate admissions between Blue Ridge Care Intake and community hospitals, providing accurate information for hospice eligibility determination.
  • Order Durable Medical Equipment (DME) as needed for patients and coordinate with the Intake Department.
  • Patient Care & Advocacy: Conduct supportive visits to patients admitted to community hospitals and communicate the patient’s status and needs to the primary care team.
  • Meet with caregivers and/or family members to share information regarding hospice services, lead family meetings, and obtain admission consents.
  • Complete Blue Ridge Care admissions within the hospital setting and act as a patient advocate.
  • Implement nursing interventions to relieve pain and other symptoms of life-limiting illnesses, delivering care within the scope of nursing practice.
  • Participate in holiday coverage, rotating at least 1–2 major or minor holidays per year among staff to ensure consistent service delivery.
  • Education & Support: Provide formal and informal education related to hospice philosophy and pain/symptom management.
  • Articulate the essential components of the hospice benefit as administered under Medicare and other payers.
Qualifications / Experience
  • Current Registered Nurse (RN) license from the State of Virginia. BSN preferred.
  • Excellent oral and electronic communication skills.
  • Proficient computer skills and electronic medical records required.
  • Must be able to travel throughout the territory as needed.
