Direct Support Professional - Adult Services
Substitute
The Direct Support Professional - Adult program participates in direct care and plan implementation for clients as prescribed by physicians and treatment teams. They provide supervision, safety and crisis management for the client population; complete documentation of client behavior and participation in programming; adhere to scheduled activities; role model positive, mature behavior; utilize proactive behavioral strategies to minimize client crisis and maximize success.
EssentialJob Functions
- Ensures the health, safety, and wellness of clients.
- Supports client health and hygiene and utilizes infection control procedures.
- Reports serious incidents, participates in reviews, and implements follow‑up measures, as applicable.
- Follows and administers the individualized plan for clients, including understanding of strengths, needs, abilities, and preferences (SNAP), goals/objectives/outcomes, and treatment strategies from current plans (Treatment Plan, IEP, ISP).
- Collaborates with the Treatment Team to provide continuity of support and care through communication and participation in team discussions and decisions.
- Promotes clients’ achievement of behavioral goals by implementing behavior strategies and teaching alternative behaviors, as indicated in the Treatment Plan/IEP/ISP.
- Actively instructs assigned clients to help them achieve educational/residential goals and objectives or r outcomes.
- Collects and documents behavioral and instructional data accurately and reliably; documents services delivered promptly.
- Maintains high engagement and monitors clients, implementing planned activities throughout the workday.
- Provides a supportive and solution‑focused environment through building trust, respect, and good relationships with clients.
- Manages behavioral crises safely.
- Oversees site management, including housekeeping, nutrition, and transportation.
- Assists with purchases and is accountable for program and client money, as applicable.
- 18 years of age.
- High school diploma or GED.
- Valid driver’s license with an acceptable driving record.
- Demonstrates moderate computer skills (login, web navigation, basic Microsoft Office, keyboarding).
- Starting at $15.50 up to $18.55 per hour based on years of experience.
- Depending on hours worked, may be eligible for Grafton's shift differential currently offered.
- Shift Differentials: $1.50/hr for weekday evenings & overnight 3:30 p.m. – 10:30 p.m.; $3.00/hr for weekends Fri 3:30 p.m. – Mon 7:00 a.m.
