The RN Case Manager works to enhance the quality of patient management and satisfaction, to promote continuity of care and cost effectiveness through integration of the functions of case management, utilization review/management and discharge planning. The RN is charged with monitoring the implementation of the plan of care as determined by the physician and works collaboratively with the multidisciplinary team to facilitate achievement of desired treatment outcomes.
Qualifications- Graduated from an accredited School of Nursing. CCM preferred.
- 3+ years experience in acute care nursing. Case management experience preferred.
- Current RN license in the state of Tennessee.
