Qualifications for Physical Therapist
  • Current Massachusetts State License as a Physical Therapist
  • Ability to work flexibly and be part of a dynamic, cooperative team
  • Experience in evaluating, assessing, and planning treatment plans for patients
  • Excellent communication skills to collaborate with residents, families, and the interdisciplinary team
  • Prior experience in geriatrics or long-term care is preferred but not required
Responsibilities
  • Evaluate and assess patients within state, payor, or facility timelines
  • Develop personalized treatment plans based on evaluations and patient needs
  • Provide hands-on physical therapy treatments for patients
  • Communicate with other team members, including nurses and physicians, to ensure patient care is coordinated
  • Document patient progress, treatments, and outcomes per regulatory and facility requirements
  • Participate in family meetings and care conferences as needed
  • Provide education to patients, families, and staff about physical therapy goals and treatments
  • Maintain accurate documentation and submit timely treatment charges
