Wiltshire Council is seeking an experienced Advanced Occupational Therapist to join the Reablement Team based in Trowbridge.
The role will focus primarily on hospital discharge work, undertaking assessments, determining care packages and support plans, and managing a large and complex caseload to maintain or improve client wellbeing in line with service procedures and national legislation.
Key Requirements:
Qualified Occupational Therapist
HCPC registered
Experience in Reablement and hospital discharge
Ability to manage complex caseloads independently
