Join our team in a 90-bed Skilled Nursing & Rehabilitation Unit, with 20 Rehab beds and 70 LTC beds. Staffed with RNs, CNAs, and Medication Aides. Operating with a patient ratio of 1:15 days, 1:19 evenings and 1:25 nights, our unit cares for patients with CHF, COPD, Diabetes, ESRD, Renal Failure, Dementia, Stroke, and Heart Disease. This role offers an opportunity to work collaboratively in a supportive environment focused on providing quality care and support to individuals requiring temporary rehab assistance and LTC needs.
Howyou'll contribute
A Registered Nurse (RN), who excels in this role:
- Accurately performs patient assessments and identifies patient needs
- Identifies and initiates appropriate nursing interventions
- Provides care appropriate to condition and age of the patient
- Performs timely and appropriate documentation relating to medical necessity
- Responsible for completion and revision of the Interdisciplinary Care Plan for each patient
- Performs timely and accurate QI assessments

A compassionate nurse with a strong foundation in providing holistic and compassionate long-term care, with a focus on detailed patient assessment, medication administration, and chronic disease management.
Requirements- Applicants should have a current state RN license and possess a degree from an accredited nursing school.
- Basic Life Support certification required within 30 days of hire.
- One year of experience preferred.
