Provides proper patient care.

• Provides and/or assists the resident in bathing and all other ADL’s as per care plan.
• Maintains appropriate toileting/bowel regimens as per individualized care planning.
• Assists with feeding residents, tray distribution, and recording of intakes as per policy.
• Responds to resident requests in a timely manner.
• Distributes nourishments and fluids, assists resident and records intake appropriately.
• Provides physical assistance to residents for daily needs.
• Provides designated exercises and repositioning for the residents.
• Maintains required documentation and food/fluid records in a timely manner.
• Receives/gives report to team members/team leaders at the stipulated time with all information complied.
• Assists in maintaining resident equipment and environment in a neat clean manner, assuring process

• Completion of NYS approved certified nursing assistant program
• Active CNA certification within New York State
• Minimum age requirement: 18

• Ability to read, write and speak English, ability to follow verbal and written instruction

M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects;
Requires frequent walking, standing or squatting.
