- Daytime clinic:
Monday to Friday, 8:00 AM – 5:00 PM with dedicated charting time. No call or weekend requirements. - Urgent or emergent situations are directed to the patient’s primary care provider.
- Average day: 10 patients – 3 new, 7 follow‑ups.
- New patient visits ~1 hour; follow‑ups 30‑minute to 1 hour based on provider discretion.
- Provide quality care in a clinic environment for patients from diverse socioeconomic and cultural backgrounds.
- Educate caregivers on disease trajectory, advance care planning, medication administration, and end‑of‑life expectations.
- Use company equipment as needed.
- Coordinate care with community medical providers and other disciplines, obtaining new orders and providing updates during and between visits.
- Collaborate with interdisciplinary team: other providers, nurses, medical social worker.
- Assist patients with symptom management for serious chronic illness, helping them live as fully as possible.
- Graduate of an accredited school with a Master’s Degree in Nursing; certification in hospice/palliative care preferred.
- Current certification from ANNNP or ANCC; current NC/SC Board of Nursing license or ability to obtain.
- Minimum five years general medical experience in inpatient, outpatient, and nursing‑home settings.
- Minimum two years as a nurse practitioner; demonstrated competence in leadership and teaching.
