- Effectively and efficiently document all coordination of resources.
- Coordinate referrals for client-specific discharge planning; ensure family participation in creating a multi-faceted discharge plan representing the family’s choice for service providers and community resources.
- Monitor record keeping, compliance, and authorized units; ensure client charts are up-to-date and include necessary documentation, including incident reports within 24 hours as needed.
- Monitor and assess documents for safety.
- Complete necessary documentation of each case in compliance with Merakey standards, county MHDS/CASSP, OMHSAS, and other licensing/regulatory/payor organizations.
- Bachelor's degree
- At least 1 year of experience working with children/adolescents
- Valid Driver's License and clean driving record
- Able to work Monday-Friday with an ON-CALL rotation

The Case Manager will:
- Coordinate resources and information pertinent to client needs.
- Effectively and efficiently document all coordination of resources.
- Coordinate referrals for client-specific discharge planning; ensure that the family actively participates in creating a multi-faceted discharge plan; one which represents the family’s choice for service providers and community resources.
- Monitor record keeping, compliance and authorized units remaining; ensure that client charts are up-to-date and include all necessary documentation, including documentation of significant incidents within 24 hours as needed (incident reports).
- Monitor/assess documents safety.
- Complete necessary documentation of each case, which complies with the standards set by Merakey, county MHDS/CASSP, OMHSAS, and other licensing/regulatory/payor organizations.
Earn $20.67/hour
