$17.00. Final compensation is based on factors such as the skills, qualifications, and experience.
Essential Responsibilities- Attend and actively participate in all training as assigned.
- Conduct community-based outreach at YMCA branches, partner sites, and community events, including Saturday.
- Build trusting relationships with community members using a culturally responsive, strengths-based approach.
- Conduct SDOH screenings and identify participant needs.
- Provide warm referrals to community-based organizations and YMCA programs.
- Support participants with navigation and follow-up to reduce barriers to services.
- Provide basic health education aligned with grant priorities and YMCA chronic disease programs.
- Accurately document services, referrals, and outcomes using HIPAA-compliant systems.
- Track grant-required metrics and support reporting and evaluation efforts.
- Perform other duties as assigned or necessary.
- High school diploma or GED required.
- Community Health Worker certification or willingness to obtain within grant timelines.
- Ability and willingness to work Saturday hours.
- Experience or lived experience working with underserved or priority populations.
- Knowledge of Social Determinants of Health and community resources.
- Strong communication, documentation, and organizational skills.
- Ability to meet grant reporting and data collection requirements.
- Multi-lingual skills.
- Experience working in grant-funded positions.
- Familiarity with Wilmington and New Castle resources.
