This role will support clients in Kent County, New Castle County, and surrounding areas, making a real difference in the lives of those we serve.

What You ll Do- Be the first point of contact for patients, introducing them to our services with warmth and professionalism.
- Complete start-of-care assessments and documentation to onboard new patients seamlessly.
- Design personalized care plans tailored to each patient s unique needs.
- Guide and instruct care team members to ensure top-quality service delivery.
- Monitor and report patient progress, keeping families and leadership informed.
- Build lasting relationships with clients, always advocating for their well-being.
- Connect with community partners and referral sources to expand our impact.
- Collaborate with clinical leadership and administrators on patient updates.
- Review caregiver documentation to maintain the highest standards of care.
- Participate in interdisciplinary team meetings to ensure holistic patient support.
- Take on additional responsibilities as needed to support our mission.
- Active Delaware RN license (required).
- At least 1 year of nursing experience (LPN-to-RN transitions welcome).
- Minimum of an Associate s Degree in Nursing.
- Strong communication skills-both written and verbal-with the ability to connect at all levels.
- Confidence in presenting and collaborating across teams.
- Reliable transportation for home visits (mileage reimbursement provided).
- Comfort with technology (computer/iPad) for documentation.
