Welcome to Our Team!
We are looking for a Board Certified Primary Care physician to join our established PACE Program. If you have a CAQ in Geriatric Medicine and at least 1 year of experience caring for the Geriatric population, we want to hear from you!
Responsibilities:- Provide primary medical care to participants
- Assess participants and develop a plan of care in collaboration with the interdisciplinary team
- Provide direct participant medical care and evaluate the effectiveness of the plan of care
- Function in a collegial relationship with other health care professionals
- Make independent decisions regarding medical needs and health regimens
- Board Certified in Primary Care
- CAQ in Geriatric Medicine
- Minimum of 1 year of experience caring for the Geriatric population
- Competitive salary of $,000
- Monday through Friday schedule with shared evening and weekend call
- Located in Wilmington, Delaware - a city with a big city feel and small-town charm
- Conveniently situated 30 minutes south of Philadelphia and midway between New York and Washington DC
